An Advanced Care Team is a group of professionals that assist in ‘enhanced care’ to your child as part of the Medical Home.  The Care Team consists of a registered nurse (Samantha) that provides health coaching as well as coordination of your child’s care (if needed), a Licensed Clinical Social Worker for counseling (Joe), and a registered nurse for triage of your child’s medical needs (Crystal).
